Warranty

We are sure you will enjoy the dentistry accomplished in our office for many years.

Because we are confident that our office provides you with the best dental treatment available, we warranty our crowns, bridges, and onlays from breakage, for a period of five (5) years from the date of delivery. If your porcelain/gold crown develops decay you will be charged the current fee for a new crown. Decay develops from plaque left on the teeth on a daily basis. Therefore excellent oral hygiene is paramount to maintain the health and integrity of your crown(s) or bridge(s).

Under warranty, if it becomes necessary, we will redo, repair, or replace them at no charge. **PLEASE REFER TO DETAILS ON BACK PAGE.**

For this warranty to remain valid, you must have completed your diagnosed treatment, including bite and splint recommendations. Also, Dr. Seaman or Dr. Wiley must see you annually for an exam & diagnostic x-rays for the same five (5) year period.
